Please complete the digital application below as thoroughly as possible and submit the required Physician Statement. The Physician Statement must be completed by your treating physician, and your application will not be processed until it is received. We recommend bringing the form with you to your next doctor’s appointment. You may also include any additional information that could assist the JCCF Board of Directors in reviewing your case.

The Justin Cowboy Crisis Fund provides need-based financial help to rodeo athletes injured while competing. Assistance is not retroactive and, if approved, will begin based on the date your completed application is received—not your injury date.
​
JCCF helps cover basic monthly living expenses (not medical bills) during a crisis. Assistance may be provided for up to 12 months. Because applications go through a thorough review process, it may take some time before you receive a decision.

Applications will not be reviewed until we receive both of the following:

The completed application form

The Physician Statement, completed by your doctor (download available above and below)
If you’ve applied to another organization for help, JCCF will wait until their decision is finalized before reviewing your case.
The Board considers:

Your history and contributions in the sport of rodeo

The nature and severity of your injury

Your ability to earn income outside of rodeo

Support from family or other sources

Your current financial situation
If approved, funds are sent via ACH direct deposit. You will be asked to complete the ACH form once approved, this is required.

Once you submit your application, you will receive a confirmation email and a copy of your application.
